How to make a referral for services:
- If you are over the age of 21 and wish to see if you qualify for services contact: Goold Health Systems Inc. at 1-800-609-7893. They may schedule an assessment to determine eligibility of services.
- If you would like to make a referral for a child under the age of twenty-one (21), please contact Care & Comfort directly to schedule an assessment to determine the child's eligibility.
- If you wish to make a Private Pay referral contact a Care & Comfort Staffing Coordinator to establish services for in-home care.
- Care & Comfort accepts referrals from community or agency providers, individuals, parents, physicians, schools, hospitals, and many Insurance companies.

If you need help Monday – Friday 8:00 a.m. – 5:00 p.m. please call the branch office nearest you. If you call after hours, you can either leave a message or page our on-call staff who will take your name and number and have the office contact you the next business day.
